Project Request Form
- This form MUST be submitted electronically in order to have your request placed into the system.
- Fill out form in its entirety. Ensure that all information is correct on form, then press “Submit”.
- After clicking “Submit”, a new window will appear to provide you with the option to print out the results of your form
- Form will be emailed directly to Campus Facilities for processing. You may print out a copy for your records if you wish.
- If form is submitted by appropriate dean/director, no signature is needed.
- If a dean/director does not email the form, it must be printed out and then signed by the appropriate person. Please send it interoffice mail to Campus Planning & Architecture, 5313.
- Any questions or problems with submitting the form, please call our office at 7-8544.
